What is the survey about?
Our survey aims to understand the assets and gaps in services and support for individuals with TBI, including concussions, and their caregivers. Your insights will directly contribute to the development of a state plan geared towards improving the quality of life for individuals across all levels of TBI severity.
How can you participate?
We estimate that it will take approximately 20-25 minutes to complete. Your responses will remain confidential.
